If you are unable to keep your appointment, please contact us by phone to cancel. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies.

Please bring the following items with you to your appointment:

  • Photo ID (required)

  • Insurance card

  • MRIs

  • X-rays

  • Please bring a current list of prescribed and over the counter medications with you including the dosage and any known drug allergies.

Fees & Payments

We accept most major insurances including, but not limited to:

Acclaim (Health Link), Aetna, Ambetter, Blue Cross Blue Shield, CHIP, Cigna, Medicare (including most Medicare advantage plans), Medicaid, Molina, MPCN, MSCAN, Tricare, United Healthcare, VA, and Workers’ Compensation.

We also accept over 600 additional insurance plans. If you have insurance, we will gladly help you file for any benefits to which you are entitled. However, it remains the responsibility of the individual patient to settle his/her account promptly. To help us file your claim correctly, we must make a photocopy or scan an image of your card(s).
